Codes & Time Thresholds

• 99492 (initial month): 70 minutes total BHCM + psychiatric consultant time.
• 99493 (subsequent month): 60 minutes.
• 99494 (add‑on): Each additional 30 minutes.
• G0512: FQHC/RHC CoCM.
• G2214: First 30 minutes option where applicable.

Required Elements (examples)

• Patient consent documented.
• Validated scale administered (e.g., PHQ‑9, GAD‑7) and reviewed.
• Psychiatric consultant case review (when required) and PCP recommendations.
• BHCM activities with start/stop times and brief summaries.

Month‑End Artifacts

• CSV minutes rollup with totals and add‑on counts.
• Summary PDF (encounters, measurements, recommendations).
